3. 1 Kids Zone Parent Pledge Form (form available on Registration Information Page)
4. Cash or Check for $65 per child.  One Check Per Child.  No Credit Cards Accepted.
5. Self Addressed Stamped Envelope

• If your player is playing this fall, update his/her record and print 3 originals of his/her form.

• If you do not see your player's record on your log on, please call the below number to have the record "linked" to your log on.
